As a promoter of success conditions at a local scale and as close to the population as possible, the Service du Développement des Pratiques Professionnelles (SDPP) of l-AVASAD responds to challenges of quality, professional support, capacity development, and innovation in the field of home care, particularly regarding health prevention and promotion. This team of 30 employees is the guarantor of an evolving and equitable offer for the entire population of Vaud. To complete the team, we are looking for a:

Social Worker (70% to 90%)

Missions and responsibilities:

• Handle requests arriving at the centrale des solidarités. This telephone line aims to offer a coordinated response as well as to promote social action throughout the Canton;
• Provide expertise and ensure a quality social response in accordance with the code of ethics defined by the profession;
• Establish a short, medium, and long-term intervention plan in situations requiring it;
• Collaborate with partners in the socio-sanitary network;
• Contribute to the creation and evolution of internal documentation to guarantee the quality of services of the Centrale des Solidarités.

Your skills and experience:

We are looking for a female or male employee who ideally has 1 to 3 years of experience in social care practice. The ideal candidate has knowledge of the medico-social environment.

The function requires:

• A Bachelor d-assistant·e social·e HES
• A strong interest in teamwork while having a great capacity to work autonomously
• A sense of detail with an analytical capacity, while being solution, client, and results-oriented
• An open, communicative, collaborative personality with ease in internal and external contacts.
